Acquiring An Emergency United States Passport Replacement

There are any number of reasons why you might need to get a passport replacement in a hurry. Your current one might have been lost or stolen. It may be out of date and suddenly something comes up and you need to go abroad. The early arrival of a baby overseas, a sudden family illness or bereavement or an urgent business matter. All of these are valid reasons why you may need to obtain an emergency U.S. passport replacement.
Report Lost Passport
A lost or stolen passport must be reported immediately. If you are within the USA, the loss may be reported over the telephone to the US Department of State using a toll-free number. If abroad, you need to contact the nearest US Embassy or Consulate. The telephone call should be followed up by completing and signing Form DS-64, which is available over the Internet from an Acceptance Facility or passport agency.
DS-64 Form
Form D-64 has to be completed using black ink and it must be legible. Answer every question as accurately and completely as you can. If you are later found to have given false information, you may be fined sent to prison. A different form is needed for a pasport renewal application.
Necessary Forms
Your next task is to complete a Form DS-11, Application for an American Passport. You will find this online or obtain it from an Acceptance Facility or Passport agency. There spaces for your name, contact detail, brief physical description, job and the name of your employer or your school. You will need to give information about your parents and if you have ever been married or used a different name. You also need to say where you are going to travel,whether you have ever had a previous passport and for your emergency contact details.
Passport Agency
You may not submit form DS-11 by mail. You must deliver it in person. Do not sign the form until asked to do so by an authorized Acceptance agent. If the passport is needed within four weeks so you can obtain a visa from another country, or within two weeks for international travel, you will need to make an apassportointment at the Regional Passport Agency.
Documents
As well as the DS-11 form, you must bring proof of American citizenship. This can be in the form of a consular report of birth overseas, proof of naturalization or a certified copy of your birth certificate. This will bear a multicolored, raised or embossed seal as well as the signature of the Registrar.
Photos
There are other pieces of documentation you will need to take with you when you go to the Regional Agency. These are a passport photograph, the relevant fee and proof of identification. Your ID may be a valid driver’s license, a government ID, naturalization certificate, military ID or a previously issued passport that has not been damaged. You should also bring a photocopy of your Proof of ID.
